About the Opportunity
We are seeking an exceptional healthcare leader to join our growing Care at Home leadership team as QLD State Manager, Care at Home .
Based in Brisbane , this is a Permanent Full-Time opportunity with responsibility for the operational leadership, growth and optimisation of Care at Home services across Queensland.
Reporting to the National Director, Operations , you will lead a diverse portfolio of innovative in-home and virtual care programs, supporting both publicly and privately funded patients across a range of clinical streams.
This role is responsible for driving operational excellence, workforce capability, clinical service delivery, stakeholder engagement and sustainable service growth across Queensland.
Working closely with executive leaders, clinicians, hospitals, funders and strategic partners, you will play a pivotal role in designing and implementing new models of care while ensuring patients receive high-quality,
safe and person-centred healthcare in their homes and communities.

Key Responsibilities
Strategic Leadership & Service Growth
Lead the delivery, growth and optimisation of Care at Home services across Queensland, including Hospital in the Home (HITH), rehabilitation, aged care, palliative care, cancer care and cardiac programs.
Partner with Product, Growth and Clinical teams to design, implement and scale innovative home-based and virtual care services.
Develop and execute state-wide operational strategies that support sustainable growth, service excellence and organisational priorities.
Build strategic partnerships with hospitals, clinicians, insurers, Primary Health Networks, government agencies and other key stakeholders.
Operational Excellence & Clinical Governance
Provide operational leadership across multidisciplinary teams, ensuring services are protected, effective, financially sustainable and aligned with clinical governance requirements.
Oversee workforce planning, service scheduling, resource allocation, performance management and operational delivery.
Monitor service performance, patient outcomes, risk, quality and financial metrics to drive continuous improvement.
Champion accreditation, compliance, quality and safety standards across all programs.
Patient Experience & Innovation
Drive patient-centred models of care that improve access, experience and health outcomes.
Lead the integration of virtual care technologies, remote monitoring solutions and innovative service delivery models.
