About the Opportunity
n
We're seeking experienced Clinical Nurses to join our expanding Hospital in the Home (HITH) service across Victoria.
n
As a HITH Clinical Nurse,
you'll deliver advanced clinical care to patients with acute and complex healthcare needs while working as part of a progressive multidisciplinary team committed to innovation, compassion, and clinical excellence.
n
You'll use your advanced nursing expertise, clinical judgement, and care coordination skills to help patients receive safe, high-quality care in the comfort of their own homes.
n
Key Responsibilities
n
n
Conduct comprehensive patient assessments and clinical reviews.
n
Develop and implement individualised care plans.
n
Deliver advanced nursing interventions including IV therapies and complex clinical care.
n
Monitor patients with acute and complex healthcare needs and elevate care when required.
n
Coordinate referrals, admissions, and discharge planning.
n
Collaborate closely with Medical Officers, Clinical Leads, and multidisciplinary teams.
n
Support patients, families, and carers through education and care planning.
n
Participate in quality improvement initiatives and service development activities.
n
Mentor and support new and junior team members.
n
Contribute to creative healthcare delivery that strengthens the connection between hospital and home.
n

About You
n
You're an experienced Registered Nurse who thrives in an autonomous environment, enjoys building meaningful patient relationships, and is passionate about delivering exceptional clinical care in community settings.
n
You Will Bring
n
n
Current registration as a Registered Nurse with AHPRA.
n
Minimum three (3)
years post-graduate nursing experience.
n
Experience in community nursing and/or acute medical, surgical, emergency, or critical care settings.
n
Demonstrated experience supporting patients with complex clinical needs and multiple co-morbidities.
n
Strong clinical assessment, care planning, and decision-making capabilities.
n
Excellent communication and stakeholder engagement skills.
n
Confidence working independently while collaborating with multidisciplinary teams.
n
Strong digital literacy and willingness to utilise telehealth and virtual care technologies.
n
Current unrestricted Victorian Driver's Licence.
n
Full and unrestricted working rights in Australia.
n

Highly Regarded
n
n
Hospital in the Home (HITH), community nursing, outreach nursing, or virtual care experience.
n
Postgraduate nursing qualifications.
n
Experience contributing to service innovation, quality improvement, or clinical projects.
n
