This is a full-time role, working business hours Monday-Friday.
This community-focused role delivers recovery-oriented outreach and community based supports across psychosocial programs such as the Bondi response team, AOD services and the National Disability Insurance Scheme (NDIS).
You will work closely with participants, psychiatrists, families, carers, and the broader community to provide holistic, strengths-based mental health support that promotes independence, stability, and wellbeing.
You will be responsible for: Conduct comprehensive mental health assessments and regular wellbeing check-ins Provide psychosocial supports focused on recovery and empowerment Undertake case management,



including care coordination and service navigation Monitor and check medications in collaboration with treating psychiatrists and GPs Develop, implement, and review individualised support plans aligned with psychosocial and NDIS frameworks Liaise with psychiatrists and other community stakeholders to ensure integrated care Advocate for participant choice, control, and community participation Maintain accurate clinical documentation and compliance records To be successful in this role, you will require: Current AHPRA registration as a Registered Nurse Postgraduate qualification in Mental Health Nursing (or working toward) highly regarded Demonstrated experience in community mental health Strong understanding of psychosocial disability and recovery-oriented practice Experience working within the NDIS framework Excellent communication and stakeholder engagement skills Current NSW Driver's Licence If you want to discuss the position further, please submit your resume to arrange an exploratory confidential call.
