Community Worker – Full-Time Total 247 Care Pty Ltd is seeking a permanent full-time Community Worker to provide community-based and in-home support services to clients in accordance with individualised care and support plans.
Key Responsibilities Provide direct support to clients in their homes and community settings Assist clients with daily living activities in accordance with individual service plans Support clients with community participation, recreational and social activities Assist with transportation to appointments and community activities Monitor client wellbeing and report concerns to management Maintain accurate client documentation, case notes and progress records Liaise with families, carers,



health professionals and community service providers Assist in implementing and reviewing client support and behaviour programs Support implementation of care plans and promote client independence and social inclusion Ensure compliance with relevant NDIS standards, policies and procedures Qualifications and Experience Certificate III or higher in Community Services, Individual Support, Disability Support, Ageing Support, Health Care Services, Caregiving or a related field At least one year of relevant experience in community services, disability support, aged care, home care, caregiving, health care support or a related field Solid communication and interpersonal skills Ability to provide person-centred support and work effectively with clients, families and multidisciplinary teams How to Apply Applications must be submitted via email to ************************ with a resume and cover letter.
