About the Role
n
As ourRN / Care Coordinator, you'll play a key role in making sure our clients receive the right care, at the right time, in the comfort of their own homes.
n
Your day-to-day will include:
n
n
Coordinating and overseeing in-home care services
n
Conducting care assessments, reviews, and service planning
n
Building strong relationships with clients, families, carers, and the wider care team
n
Monitoring care delivery to ensure quality, safety, and compliance
n
Supporting carers and staff withguidance, training, and regular updates
n
Travelling throughout theWide Bay and surrounding areas— with acompany car provided for work-related travel
n
n
What We're Looking For

We're looking for someone who iscaring and capable, who genuinely enjoys connecting with people and takes pride in making a difference.
n
You'll ideally bring:
n
n
Previous experience in Aged Care, Community Care, or Disability Support
n
Strong time management and organisational skills
n
Excellent communication and interpersonal skills
n
Knowledge ofcare standards, assessments, and service coordination
n
A current driver's licenceand access to a reliable vehicle
n

Qualifications
n
n
AHPRA Registered and practicing
n
Drivers Licence
n
Any other Aged Care / Health Care related certificates
n
Proven experience in aged care, disability, or mental health within a community setting.
n
Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
n
Current First Aid, CPR certification, and National Police Check.
n
Working With Children (WWC) check or willingness to obtain.
n
A valid driver's license and access to a reliable car with comprehensive insurance.
n
