The role reports to the In-Home Care Team Leader and focuses on comprehensive needs assessments and care plan development to support participants living at home.
The successful candidate will be an AHPRA-registered RN with aged or community care experience, capable of coordinating services across multidisciplinary teams and maintaining confidentiality.
