Our Clinical Care Coordination services provide intensive, wrap-around supports for people experiencing severe and persistent mental health challenges, bringing together clinical care, psychosocial support, and access to essential community services.
By integrating clinical care, psychosocial support, and access to essential community services, we aim to provide holistic, person-centred care that promotes recovery and wellbeing.
Clinical Care Co-ordination is part of the Better Connect initiative, involving eight partner organisations.
Better Connect delivers coordinated, person-centred mental health, alcohol and other drugs (AOD), early intervention, psychosocial, and suicide prevention support across Central Queensland, Wide Bay and the Sunshine Coast.
This provides a valuable opportunity for collaboration and working in partnership.
We're seeking a compassionate and skilled Clinical Care Coordinator in Rockhampton to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.
Working closely with a multidisciplinary team, you'll support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.




Responsibilities Conducting clinical assessments and developing tailored treatment plans Deliver evidence-based clinical and psychosocial interventions Managing caseload and conducting risk assessment with management plans Collect data and supporting monitoring and evaluation Collaborate with teams and external stakeholders Facilitate care planning and review processes Monitor and escalate mental health needs as appropriate Perks Advantages Access to salary package up to $15,900 and $2,650 for meal and entertainment p/a of tax-free income, remote area benefits and additional relocation assistance Access to professional development opportunities Access to supportive clinical supervision and EAP Competitive pay, discounts at hundreds of retailers and referral bonuses Requirements Tertiary qualification in Psychology, Social work, Mental Health Nursing, or similar Current Mental Health Accreditation with AHPRA Registration or eligibility for AASW Membership Experience working with a trauma informed, recovery focused approach supporting people with severe mental health challenges Experience working with diverse cultures, especially First Nations people Strong teamwork and collaboration skills, problem-solving and organisational abilities About Open Minds Open Minds has delivered mental health services across NSW and QLD for over 114 years, including headspace centres, Medicare Mental Health Centres, universal aftercare, psychosocial programs, and clinical care coordination.
