Gem at Home is seeking a dynamic junior doctor who is eager to make a difference in the lives of our patients. As a Gem at Home junior doctor, you will have the opportunity to work closely with our Advanced Trainee and Geriatrician who will provide mentorship and support as you navigate the exciting world of community-based healthcare.
n
In this role, you will have the flexibility to choose your working days (if part-time), allowing you to balance your skilled and personal commitments. You will be based at our office/hub located at Sunshine Hospital, where you will be part of a supportive and collaborative team dedicated to delivering high-quality care to our patients.
n
Gem at Home:
n
n
Aiming to enable earlier discharge of patients over the age of 65 whose family and/or carers can support them at home whilst they continue to make functional and cognitive improvements following hospital admission
n
Provides comprehensive Geriatric care and assessments from the patient's home. Referred by Western Health acute and subacute wards via SNAP (Subacute and Non-acute Assessment and Pathways) and external healthcare networks
n
Up to 20 virtual beds with a length of stay varying from 1-4 weeks depending on patient's progress
n
n
Roles available:
n
n
Full Time - Monday to Friday (no weekends)
n
Part Time - Monday to Friday (working days are negotiable)
n
n
Key responsibilities:
n
n
Providing patient care as part of a multidisciplinary team
n
Conducting phone calls(including telehealth)to patients and other specialties/GPs
n
Liaising with inpatient teams
n




Completing admissions and clinical reviews in the patient's home and completing discharge summaries in a timely manner
n
You must have an Australian Driver's license and be willing to perform home visits with support from our team
n

About you:

Current APHRA Registration as a Medical Practitioner
n
Post-graduate Year 3 or above at the time of commencement
n
Valid Australian driver's license
n
Commitment to ongoing professional development and quality improvement
