Mater Hospital Brisbane is seeking an experienced Registered Nurse to join the Planned Care team in the Mater Referral Hub.
This permanent part-time role is based at South Brisbane, working six days per fortnight.You will screen referrals, assess information, and direct referrals to the appropriate service or care pathway.
As part of a multidisciplinary team, you will deliver secure, efficient, patient-centred care, liaising with referrers, patients and clinical colleagues while applying referral
