Apply now Job no: ******Work type: PermanentLocation: South BrisbaneCategories: Registered Nurse/MidwifeAbout the roleMater Hospital Brisbane is seeking an experienced, motivated and enthusiastic Registered Nurse to join our Planned Care team in the Mater Referral Hub.
This is a permanent part-time opportunity based at South Brisbane working 6 days per fortnight.The Mater Referral Hub receives and processes all public outpatient referrals, for Mater Hospital Brisbane and Mater Hospital Springfield.
New patient referrals are received from various sources (predominately GP and specialist) and are clinically screened using endorsed guidelines and are registered and waitlisted on the relevant (Mater) facility's waiting list.As a Registered Nurse within the Mater Referral Hub, you will play a key role in supporting patient access to care through the timely processing and screening of referrals.
Working as part of a multidisciplinary team, you will use your clinical knowledge and judgement to review referrals, gather and assess relevant information, and ensure referrals are directed to the most appropriate service or care pathway.You will contribute to the delivery of safe, efficient and patient-centred care by applying established referral management processes, liaising with internal and external stakeholders, and supporting equitable access to services.
As an important point of contact for referrers, patients and clinical teams, you will help facilitate timely access to care while maintaining a high standard of customer service.About youThe successful candidate will be hard working, enthusiastic and able to show initiative.




You will be approachable with a mature and confident attitude and have the ability to work and achieve results independently.Current AHPRA registration (no restrictions) with 3+ years acute nursing experienceClinical competency and the ability to maintain the highest standards of nursing practiceExcellent knowledge and assessment skillsDemonstrated ability to work both independently as well as part of a multi-disciplinary teamHigh level communication skills when liaising with patients, families and staff within professional, legal and ethical parametersExceptional time managementHigh level competence in using electronic information systems such as patient information systems, procedural booking system, Outlook and Microsoft ExcelExisting knowledge of the ability to acquire knowledge in Mater's referral management guidelines including Clinical Prioritisation Criteria and Health PathwaysAn existing understanding or ability to acquire knowledge in applying the Specialist Outpatient Services Implementation StandardAn existing understanding or ability to acquire understanding of the national quality and safety standardsMater is Queensland's largest not-for-profit network of public and private hospitals and healthcare services, bringing together world-class expertise across healthcare , education , fundraising & philanthropy and research .
