About the Role
Join CO‑ACT at Hampstead Rehabilitation Centre and help older people achieve safe, meaningful transitions from hospital to community care.
- • Assess and deliver tailored physiotherapy for complex patient needs and support complex discharge planning
- • Manage a varied caseload with growing clinical independence
- • Partner with multidisciplinary teams to support quality patient care
- • Educate colleagues and provide clinical guidance to less experienced staff
- • Contribute to quality improvement, research and community care planning
About You
We’re looking for a skilled physiotherapist who brings sound judgement, teamwork and a commitment to quality patient care.
- • Physiotherapy degree eligible for AHPRA registration
- • Experience in acute or rehabilitation settings with adults
- • Sound clinical judgement managing complex caseloads independently
- • Strong communication, teamwork and evidence based practice skills
- • Commitment to professional development, service quality and safety
