Northside Community Service in Canberra is seeking a Care Partner (Registered Nurse) to deliver in-home nursing and care coordination under the Support at Home program, helping older people stay secure, connected and independent in their own homes.

You will work within your AHPRA-defined scope of practice, undertake assessments, develop and review care plans, coordinate services, monitor risks and provide clinical guidance to care partners and junior staff, in a supportive, community-based setting.
