Overview

SA Health's Eyre and Far North Local Health Network (EFNLHN) is looking for an experienced Home Care Coordinator to join Country Health Connect in Streaky Bay on a part-time basis, 60 hours per fortnight, until 31 December 2026.

What You’ll Be Doing

Working within the Community and Allied Health team, you’ll coordinate Commonwealth-funded aged care programs - including Support at Home and the Commonwealth Home Support Program - helping older people in Streaky Bay and surrounds remain living safely and independently. Your key responsibilities will include:

- Conducting holistic assessments and developing, implementing and reviewing person-centred care plans
- Coordinating referrals and services across health, community and government providers
- Advocating for clients while actively promoting independence, choice and self-advocacy
- Contributing to service improvement through data collection, evaluation and quality initiatives

What You’ll Bring

- Demonstrated experience in case management and coordinating care for people with complex or multiple needs
- Sound knowledge of Commonwealth aged care programs, including Support at Home or CHSP frameworks
- Ability to conduct thorough assessments and translate findings into practical, client-led care plans
- Strong communication skills with experience working across multidisciplinary teams and community networks
- Confidence managing documentation, reporting and competing priorities in a regional or rural health setting
- A non-judgemental,
