About The Role
As an Independent Living Coach, you will support participants experiencing dementia, age-related frailty, and complex health needs to maintain their independence, optimise wellbeing, and engage in meaningful daily activities. The role works closely with participants, families, nursing staff, allied health professionals, and support workers to promote quality of life and support successful transitions to long-term aged care.
About You
We are seeking a passionate and dedicated team member with demonstrated experience in aged care, a strong work ethic, and the ability to build positive and respectful relationships with participants and team members.
In Addition, You Will Be Able To Demonstrate
- Minimum Certificate III or above in Individual Support
- Experience implementing person‑centred support plans and behaviour support plans
- Experience in supporting people who have complex requirements, including behavioural, medical, or mental health support needs
- Flexibility to work various shifts including both short and long shifts on weekdays, weekends, evenings, nights, sleepovers and public holidays
- Proven ability to work both autonomously and in a team environment and meet legislative and internal policy and procedural requirements
You will also need to hold (or have the ability to obtain) a current Ochre Card, Criminal History Check, NDIS Worker Screening Clearance, First Aid and CPR certificate and an unrestricted NT Drivers Licence.
