Job Description

We are looking for a special kind of person - a St Vincent's kind of person. Someone who puts compassion, connection and caring for other people at the heart of everything they do. As an Enrolled Nurse you’ll be part of an experienced, engaged and dedicated team helping to create a welcoming, qualified and supportive care experience for the residents of St Vincent's Care Services Werribee. You will ensure the provision of quality resident care through clinical nursing practice, promoting collaborative relationships amongst the health care team. Key Responsibilities

- Delivers person centred care that coordinates individualised care from admission and throughout their stay, incorporating planning of care in collaboration with Registered Nurse.
- Provision of detailed documentation of a systematic comprehensive and accurate assessment of residents.
- Evaluates the documentation concerning resident outcomes.
- Enhances the dignity and integrity of residents and upholds their rights.
- Maintains confidentiality and ensures that resident information and records are not disclosed or misused in any way.
- Coordinates and implements the intervention(s) identified in the plan of care/clinical path.
- Evaluates and records progress towards expected outcomes and revises the care plan as necessary.
- Communicates resident response to treatment and care and documents accordingly.
- Facilitates communication between the resident, family, medical, nursing, ancillary and allied health staff.
- Referrals to be made in collaboration with RN for the purpose of comprehensive care to resident and families.
- Participate in clinical risk management, resident safety and continuous quality improvement activities as part of day-today work.




- Acts as a preceptor for new Personal Care staff.
- Carry out all other reasonable duties as directed by Registered Nurse and/or Residential Care Services Manager.

Qualifications

- Current registration with AHPRA as an Enrolled Nurse Diploma of Nursing or equivalent.
- Demonstrated knowledge of the Aged Care Quality Standards and person-centred care principles.
- Medical administration competency. NDIS Worker Screening Check may be a requirement of this role.

Contract & Working Hours

Full Time/Part Time positions available. Strong shift availability is essential.
