Silverchain is seeking a Care Partner to join the Sunshine Coast region for a 12-month fixed term. You will visit aged care clients in their homes, manage caseloads and collaborate with clinical and service delivery teams to ensure holistic care.

You will assess needs, develop personalised care plans, manage funding budgets in line with government guidelines, and onboard recent clients with a compassionate, person-centred approach. Experience in community care and advocacy is valued.
