South West Community Care (SWCC) is a local not-for-profit organisation and registered charity providing in-home aged care services across the South West.
Our Purpose: To help older people live safely, independently, and with dignity in the place they love – their own home.
We are governed by a volunteer Board of Governance and led by our Chief Executive Officer, supported by a Chief Organisational Officer. Together, they support more than 120 dedicated employees and volunteers who deliver care throughout the region. Our services follow the Wellness approach, focusing on independence, choice and enabling people to stay connected to their community.
We currently support around 1,200 clients from Yarloop to Capel, offering services that maintain safety, wellbeing, and quality of life.

Position Purpose
Reporting to the Operations Manager, the purpose of your role is:
The Clinical Care Manager provides senior clinical and operational leadership for secure, person-centred CHSP and Support at Home services. Working under broad direction, the role leads clinical governance, multidisciplinary care management, complex risk and continuous improvement, ensuring compliant service delivery and supporting older people to remain independent at home.
Position Responsibilities
As a Clinical Care Manager you will be responsible for and assisting with:
- Provide visible clinical leadership across CHSP and Support at Home services, ensuring service delivery reflects dignity, choice, supported decision-making,
cultural safety, wellness, reablement, restorative practice and contemporary person-centred care.
- Lead and provide professional oversight to Care Partners, Care Coordinators and clinical personnel through supervision, coaching, workload and caseload oversight, capability development, clinical guidance and accountable performance management.
- Oversee intake, clinical triage, assessment, care planning, reviews, transitions and exit processes, ensuring service responses are prioritised according to assessed need, urgency, clinical and safeguarding risk, client preference and available program resources.
- Establish and monitor standards for care planning, risk assessment and clinical documentation, ensuring client records are accurate, proportionate, current and reflective of assessed needs, preferences, strengths, goals and informal supports.
- Lead and maintain clinical governance systems for the identification, assessment, management and escalation of clinical risk, including deterioration, medication support, falls, wounds, nutrition and hydration, continence, infection prevention and control, restrictive practices, cognitive impairment, mental health and end-of-life care.
- Provide senior clinical advice and decision-making support for complex and high-risk matters, including safeguarding concerns, changes in condition, service refusals, complaints, hospital transitions and circumstances involving competing rights, risks, preferences or duty-of-care considerations.
- Provide governance oversight of incidents, near misses, complaints and clinical concerns, ensuring appropriate investigation, escalation, corrective action and closure, with identified trends and lessons translated into improvements in systems and practice.
- Monitor and evaluate clinical quality, client outcomes, quality indicators, documentation and service delivery, directing timely corrective action where care is unsafe, ineffective, delayed, inconsistent or otherwise presents unacceptable risk.
- Ensure employees, contractors and associated providers delivering care operate within appropriate scope, competence and authorisation requirements, and work collaboratively with operational and scheduling teams to align workforce capability and continuity with client needs.
- Oversee care management and multidisciplinary coordination under Support at Home and service delivery under CHSP, ensuring services and resources are appropriately utilised, program requirements are met and client outcomes demonstrate benefit, independence and reablement.
- Review and authorise clinically relevant referrals, purchases and service variations within delegated authority, ensuring decisions are clinically justified, necessary, reasonable and consistent with client needs, program requirements and available funding.
- Analyse clinical, caseload, service, incident, complaint,
quality and client outcome data to identify trends, emerging risks, unmet demand and improvement priorities, and provide evidence-based advice and recommendations to Executive Management.
- Lead clinical quality and continuous improvement activities, including organisational policy and practice development, audit and regulatory preparedness, stakeholder engagement and senior clinical escalation or on-call responsibilities within organisational delegations.
To thrive in this role you will have:
- Tertiary qualification in Nursing, with current unrestricted registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), and demonstrated experience within aged care, community health or in-home care services.
- Demonstrated understanding of contemporary aged care practice and the delivery of person-centred care that promotes client choice, dignity, independence, wellbeing and quality of life, while balancing individual preferences with identified clinical needs and risks.
- Demonstrated experience managing, supervising, coaching and developing clinical employees, including providing professional guidance, supporting capability development, addressing performance and practice concerns and promoting accountability for the delivery of high-quality care.
- Demonstrated experience in the development, implementation, review and evaluation of individualised care plans, with the ability to coordinate clinical services and ensure care arrangements remain responsive to clients’ assessed needs, goals and changing circumstances.
- Demonstrated experience responding to clinical incidents, changes in client condition and emerging clinical risks, including coordinating immediate operational responses, supporting employees and clients, implementing appropriate actions and escalating significant matters in accordance with organisational requirements.
- Demonstrated ability to work collaboratively with clients, families and representatives, internal employees, general practitioners, allied health professionals, hospitals and other external health providers to coordinate care and achieve appropriate clinical and client outcomes.
Office Base Location
You will be required to work from all SWCC sites (Dalyellup, Australind and Harvey).
