We are seeking an experienced Care Manager to lead clinical operations within a large aged care home in Melbourne's south-east, working closely with the Residential Services Manager to deliver high-quality, person-centred care and support a dedicated care team.

About the Role

As Care Manager, you will provide strong clinical leadership and oversight across the service, ensuring high standards of care, compliance and resident wellbeing.

Your key responsibilities will include:
Leading, supporting and developing clinical and care teams
Ensuring resident assessments, care plans and clinical documentation are accurate, current and compliant
Overseeing medication management and the delivery of high-quality clinical care
Providing coaching, mentoring and education to care staff
Driving continuous quality improvement initiatives and supporting accreditation outcomes
Drive best-practice clinical governance, compliance, health, safety and infection prevention.

About You

To be successful in this role,



you will be an experienced clinical leader who combines strong nursing knowledge with a genuine passion for aged care.

You will have:
Current AHPRA registration as a Registered Nurse (Division 1)
Previous experience in a clinical leadership role within residential aged care
Strong knowledge of the Aged Care Quality Standards, clinical governance and compliance requirements
Demonstrated ability to lead, coach and develop care teams
Excellent communication, interpersonal and stakeholder management skills
Robust organisational and problem-solving skills
A genuine commitment to delivering exceptional, person-centred resident care

Desirable:
Postgraduate qualifications in Gerontology, Aged Care Management or a related discipline
