Silverchain is seeking a Care Partner in Australia to deliver person-centred care, supporting independence and wellbeing for clients. The role is full time and permanent with a hybrid work arrangement between home and our offices in Park Holme or Elizabeth South.
You will conduct strengths-based assessments, coordinate services and manage budgets, collaborating with health professionals and carers to help clients stay secure and independent at home.
