Monash Health in Melbourne's south-east is seeking an experienced Grade 2 Social Worker to join our Community Rehabilitation team on a part time ongoing basis, predominantly based at Kingston Centre in Heatherton.You will deliver short-term, goal-focused psychosocial assessment and intervention within an interdisciplinary team across neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation, with supervision and student education as part of your role.
