Launch Housing in Melbourne is seeking a Northern Outreach Case Manager to deliver assertive outreach and recovery-based planning for people experiencing homelessness in the City of Yarra area.
You will work with clients in various settings, coordinate housing, income entitlements, and referrals, while maintaining records and collaborating with partner agencies.
This is a permanent full time role based in Collingwood.
