Mater Hospital Brisbane is seeking an experienced Registered Nurse to join the Planned Care team in the Mater Referral Hub.
This permanent part-time role is based at South Brisbane, working six days per fortnight.
You will screen referrals, assess information, and direct referrals to the appropriate service or care pathway.
As part of a multidisciplinary team, you will deliver secure, efficient, patient-centred care, liaising with referrers, patients and clinical colleagues while applying referral
