Salary:$53- $*****ph+Super + Not-for-Profit Salary Packagingup to $18,550

We're looking for a dynamic individual to be thefirst point of contactfor clients referred through the Head to Health Phone Line and other pathways. In this role, you'll be akey playerin guiding clients through a smooth and welcoming entry into our services, providing invaluable support on their journey to well-being.
n
Although this is not a traditional leadership role with direct reports,you'll be a leaderin spirit, overseeing the care journey and collaborating with various teams to ensure each client receives the highest quality support.
n
Responsibilities include:
n
n
Lead the way for clients: Provide direct engagement and brief interventions to ensure smooth transitions and aftercare.
n
Collaborate: Work closely with internal teams and external partners to deliver comprehensive care.
n
Support clients: Be their first contact and help them navigate their care journey, ensuring a welcoming experience at every step.
n
Manage Care Processes: Oversee collaborative care planning, transitions between services, and aftercare.
n
Strengthen Partnerships: Build relationships with local hub partners and the lived experience workforce to foster community-driven care.
n

The ideal candidate will have:
n
n
Strong leadership: You'll take charge of client care and team coordination, ensuring clients receive the best possible outcomes.
n
Experience: Extensive background in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.
n
Community-focused: Experience working independently and as part of a care team in community-based recovery models.
n
Collaboration Skills: Proven ability to engage and collaborate with diverse services and professionals in the mental health space.
n

Qualifications
n
n
Qualifications in psychology, social work, mental health nursing or a related field.
n
A maintained relevant annual registration and/or membership of professional body(preferred).
n
Current state-based driver's licence.
