Position Overview: Care Partner (Registered Nurse)
We are seeking an experienced and compassionate
Care Partner (Registered Nurse)
to join our Aged and Community Care Programs team.
This role provides care coordination and clinical nursing support to participants under the
Support at Home program
, helping older people remain safe, connected and independent in their own homes.
As a Care Partner, you will work within your AHPRA-defined scope of practice to deliver high-quality clinical care, undertake assessments, develop and review care plans, coordinate services, monitor risks, and provide clinical guidance to care partners and care workers.
This is a rewarding opportunity for a Registered Nurse who enjoys community-based practice, person-centred care, and working collaboratively with participants, families, carers and multidisciplinary teams.
What you'll be doing
In this role, you will:
Deliver evidence-based nursing care in clients' homes, including wound care, medication management and chronic disease support.
Conduct clinical and goal-oriented assessments, risk assessments and care planning.




Coordinate services, referrals and onboarding activities in line with approved care plans and program requirements.
Monitor participant wellbeing, identify changes in condition or emerging risks, and elevate concerns where required.
Support participants to understand their services, supports and approved budgets.
Educate clients and families on treatment plans, self-care strategies and reablement approaches.
Provide clinical guidance, delegation and supervision to non-clinical care partners, care workers and junior clinical staff.
Maintain accurate documentation, case notes and records using care management systems.
Contribute to quality improvement, compliance, incident reporting and clinical governance activities.
Desired Skills and Experience
About you
You will be a confident and values-driven Registered Nurse with strong clinical judgement, excellent communication skills and a genuine commitment to supporting older people in the community.
You will bring:
Current, unrestricted registration as a Registered Nurse with AHPRA.
Extensive post-registration experience in aged care, home care or community health.
Experience in goal-oriented care coordination,



care planning and ongoing monitoring.
Robust clinical decision-making, critical thinking and risk identification skills.
Excellent written and verbal communication skills.
A commitment to person-centred, wellness and reablement-focused care.
Understanding of aged care funding models, the Strengthened Aged Care Quality Standards and the Aged Care Act ****.
Confidence using Microsoft Office and client management systems.
Current CPR certification.
Current Australian driver's licence, reliable insured vehicle and mobile phone.
Current National Police Check and ACT Working with Vulnerable People registration, or willingness to obtain/maintain these.
