About the Role
We currently have two exciting opportunities available for Clinical Care Coordinators.
Located in the heart of Horsham Lutheran Homes Group's 76-bed residential care home, dedicated to delivering high-quality, person-centred care within a supportive community setting.
We are seeking two Clinical Care Coordinators to join our team.
These rewarding roles combine hands-on resident care with clinical leadership responsibilities, offering an excellent opportunity to further develop your skills and advance your career.
Working closely with the General Manager and Care Manager, you will play a key role in leading clinical excellence, supporting care teams, and ensuring residents receive safe, individualised care through a strengths-based approach.
Key Responsibilities
- Collaborate with key stakeholders to enhance residents' quality of life and health through solid clinical governance.
- Support and guide the care team to promote a collaborative culture focused on excellence and continuous development.
- Actively participate in quality and risk management to ensure compliance with standards and drive continuous improvement.
What You Will Need
- Bachelor’s degree in nursing (or equivalent)
- Current AHPRA registration required.
- Previous experience as a Clinical Nurse or Care Coordinator in aged care or a similar setting is highly valued.
- Strong clinical assessment and documentation skills are essential
- Demonstrated leadership qualities are highly regarded.
Candidates must hold a current National Police Clearance. In line with current health guidelines for the aged care sector, candidates are strongly encouraged to be up to date with all recommended vaccinations.
