We are seeking a Diabetes Educator to work in a part time (16 hours/fortnight) capacity based at Nhill.

RENUMERATION:

- Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024-2028.
- Estimated salary range as per award - $25,600 - $26,969.

ABOUT THE ROLE:

- To provide Diabetes Education services to acute hospital inpatients, community clients and aged care residents.

KEY SELECTION CRITERIA

Essential:

- Have effective communication and organisational skills.
- Qualification as either a Registered Nurse, Accredited Practising Dietitian or Accredited Exercise Physiologist eligible for membership of the Australian Diabetes Educators Association and credentialling.
- Diabetes Educators Certificate.
- Credentialled Diabetes Educator or working towards becoming credentialed.
- Demonstrate Diabetes knowledge and skills.
- The ability to work independently but also as a member of a team.
- Excellent communication (written and verbal) and interpersonal skills, including the ability to prepare and deliver education to patients, families and staff.
- The willingness to work in a rural setting.

Desirable:

- Have previous experience working in a Diabetes Educator position in hospital/healthcare setting.
- Experience and understanding of issues specific to provision of services in rural and remote areas.
- Awareness of the Accreditation process.
- Solid understanding of the Primary Health funding guidelines.
- Competence in using computer programs including Microsoft Word,



Excel, Outlook and other databases such as iCare and Uniti. Experience utilising the My Aged Care (MAC) portal is also desirable.
- Demonstrate the flexibility to manage a diverse caseload.

PRE-REQUISITES:

- NDIS Worker Screening Clearance.
- Working with Children Check.
- Fully vaccinated (including influenza) as per current Department of Health Directives.
- Australian Driver’s Licence.

KEY RESPONSBILITIES:

- Effectively manage a Diabetes Education caseload across acute, aged care and community settings.
- Actively contribute as a team member to the effective operation of the Diabetes Education Department, including attendance at regular team meetings.
- Work effectively and collaboratively within the broader WWHS and Community Health Team context.
- Establish and maintain positive relationships with external
agencies/organisations and strategic partners in relation to effective delivery of Diabetes Education Services and community development across the Wimmera.
