Join a supportive team in Rockhampton as a Social Worker and build your mental health experience while working towards your Mental Health Accreditation. You'll receive structured supervision, ongoing training, and professional development opportunities over the two-year accreditation process while managing a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.

Clinical Care Co-ordination is part of the Better Connect initiative, involving eight partner organisations. Better Connect delivers coordinated, person-centred mental health, alcohol and other drugs (AOD), early intervention, psychosocial, and suicide prevention support across Central Queensland, Wide Bay and the Sunshine Coast. This unique partnership provides excellent opportunities to learn from experienced professionals and work alongside a multidisciplinary team.

Responsibilities

- Conducting clinical assessments and developing tailored treatment plans
- Deliver evidence-based clinical and psychosocial interventions
- Autonomously manage caseload and conducting risk assessment with management plans
- Collect data and supporting monitoring and evaluation
- Collaborate with teams and external stakeholders
- Facilitate care planning and review processes




- Monitor and escalate mental health needs as appropriate

Requirements

- Tertiary qualification in Social Work with eligibility for AASW Membership and ability to obtain your Mental Health Accreditation
- Experience working with a trauma informed, recovery focused approach supporting people with severe mental health challenges
- Experience working with diverse cultures, especially First Nations people
- Strong teamwork and collaboration skills, problem-solving and organisational abilities
