This is a supportive, established team with separate scheduling and support worker functions, allowing you to focus on client relationships, care planning and service coordination rather than rostering.
You will work alongside care and scheduling professionals who understand the demands of community aged care and are committed to maintaining responsive, person-centred services.
The exciting chance:
You will take responsibility for an allocated group of clients across the northern metropolitan suburbs, helping to provide continuity of care while strengthening capacity across the team. The role is ideal for someone who can step into an existing caseload, prioritise effectively and become productive quickly.
- Manage an allocated caseload of approximately 40–45 clients.
- Complete client contact and face-to-face home visits.
- Review and update care plans in line with changing client needs.
- Apply your Support at Home knowledge across client services and funding requirements.
- Liaise with nurses, external providers, families and other relevant stakeholders.
- Coordinate with the scheduling team and support worker team to maintain service continuity.
- Identify priorities across your caseload and manage your time accordingly.
- Monitor client needs and escalate concerns when appropriate.
- Work independently while remaining connected to the wider care team.

About you:
- Demonstrated experience in Support at Home is essential.
- Previous experience in community aged care, home care or a similar client-focused setting.
- Experience managing a client caseload and maintaining individualised care plans.
- Strong understanding of Support at Home requirements and service delivery.
- Excellent time-management and prioritisation skills.
- Ability to assess a caseload, identify urgent needs and plan your work with minimal direction.
- Strong communication and stakeholder-management skills.
- Ability to work effectively with nurses, scheduling teams, support workers and external providers.
- A practical, organised and solutions-focused approach.
- Experience using eCase will be highly regarded, although it is not essential.
- Current driver’s licence and access to a reliable vehicle.
- Full Australian working rights and the ability to meet required aged care clearances.
