ACH Group is seeking a passionate Community Connector to lead our Social Experiences team and support older people to build social connections and engage with their communities.
This full time role is based at Elizabeth on Mondays and across ACH Group locations for the rest of the week, involving travel between residential care homes to backfill as needed. You will develop social plans and run engaging activities to enhance wellbeing and reduce isolation.
