What you’ll be doing.

In this meaningful frontline role, you’ll help people who have experienced homelessness or housing instability build confidence, maintain their tenancy and feel more connected to their community. Using a trauma-informed, strengths-based and person-centred approach, you’ll provide practical support, advocacy and case management that empowers clients to work toward greater independence and long-term housing stability. Some of your key responsibilities are, but not limited to:

Support clients to sustain their tenancy, build independence and thrive.

Deliver tailored, person-centred case management and support planning.

Identify risks early and provide practical support, advocacy and referrals.

Connect clients with the right housing, health and community services.

Maintain quality case records and ensure compliance and reporting requirements are met.

Manage risk and respond confidently to complex client needs and situations.

Build strong partnerships across CHL and the broader community sector.

Champion trauma-informed, strengths-based and culturally safe practice.

Contribute to a positive team culture and continuous improvement.

What we’re looking for

We’re looking for someone who is empathetic, resilient and passionate about supporting people to achieve sustainable housing outcomes. You’ll bring:

Experience supporting people experiencing homelessness, housing instability or complex needs.

Holds relevant certificate IV / Diploma in Community Services, Social Work, Human Services or related fields or equivalent experience.

Robust communication, engagement and relationship-building skills.

Experience in case management, assessment and client documentation.

A sound understanding of trauma-informed, strengths-based and person-centred practice.

The ability to work independently, manage competing priorities and use sound judgement.

Confidence working in the community,



including outreach and home visits.

A commitment to culturally safe, inclusive and respectful service delivery.

Knowledge of housing, homelessness and community support pathways will be highly regarded.
