About the Role
As ourRN / Care Coordinator, you’ll play a key role in making sure our clients receive the right care, at the right time, in the comfort of their own homes.
Your day-to-day will include:
- Coordinating and overseeing in-home care services
- Conducting care assessments, reviews, and service planning
- Building strong relationships with clients, families, carers, and the wider care team
- Monitoring care delivery to ensure quality, safety, and compliance
- Supporting carers and staff withguidance, training, and regular updates
- Travelling throughout theWide Bay and surrounding areas— with acompany car provided for work-related travel
What We’re Looking For
We’re looking for someone who iscaring and capable, who genuinely enjoys connecting with people and takes pride in making a difference.
You’ll ideally bring:
- Previous experience in Aged Care, Community Care, or Disability Support
- Strong time management and organisational skills
- Excellent communication and interpersonal skills
- Knowledge ofcare standards, assessments, and service coordination
- A current driver's licenceand access to a reliable vehicle
- A genuine commitment to our purpose: “Improving the quality of life for those we serve.”

Qualifications
- AHPRA Registered and practicing
- Drivers Licence
- Any other Aged Care / Health Care related certificates
- Proven experience in aged care, disability, or mental health within a community setting.
- Robust understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
- Current First Aid, CPR certification, and National Police Check.
- Working With Children (WWC) check or willingness to obtain.
- A valid driver's license and access to a reliable car with comprehensive insurance.
Preferred Skills
- Knowledge of Home Care Packages (HCP) or Support at Home (SAH) (Desirable)
