We're looking for an experienced and compassionate Support Coordinator who is committed to achieving positive outcomes for participants and their families.

About the Role

n

This is a dedicated Support Coordination role focused on helping participants understand, implement and maximise the benefits of their NDIS plans. The role is centred on delivering high-quality, participant-focused support, building strong relationships, and assisting participants to achieve their goals and access the services they need.

What You'll Be Doing

n

n
- Supporting participants to understand and implement their NDIS plans
n
- Coordinating services and connecting participants with suitable providers
n
- Building strong relationships with participants, families, and stakeholders
n




- Promoting independence, choice, and capacity building
n
- Monitoring budgets and ensuring NDIS compliance
n
- Preparing reports and supporting plan reviews
n
- Advocating for participant goals and positive outcomes
n

n

What You'll Bring

n

n
- Experience in NDIS Support Coordination and/or case management
n
- Strong understanding of the NDIS and relevant frameworks
n
- Excellent communication and relationship-building skills
n
- Strong organisational and time management abilities
n
- Qualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)
n
- Confidence using Microsoft Office and case management systems
n
- NDIS Worker Screening Check
n
- Working With Children Check (WWCC)
n
- National Police Check
n
- Current Driver's Licence and reliable vehicle
n
