About the Role The position supports South Pacific Private’s mission of changing lives and healing families by providing both operational leadership within the Day Programs and direct client support through Continuing Care Case Management. Working closely with multidisciplinary teams, the successful candidate will help coordinate the delivery of addiction, trauma and mental health recovery programs, ensuring clients remain engaged in treatment and receive high-quality, person-centred care throughout their recovery journey.

As Day Program Co-ordinator and Continuing Care Case Manager, you will play a key role in program administration, client retention, case management, and service quality improvement. You will support clients from their inpatient admission through the first 90 days of recovery, facilitating treatment planning, coordinating care across teams, and helping individuals and families build strong foundations for long-term wellbeing and sustainable recovery.

Key Responsibilities

Day Program Co-ordinator

- Support the Day Program Manager in the operational coordination and administration of South Pacific Private’s Day Programs and Beachwood Recovery Home.
- Coordinate client admissions, scheduling, group allocations, and multidisciplinary team processes to maximise engagement, retention, and program outcomes.
- Act as a key liaison between clinical and administrative teams, ensuring effective communication and smooth program delivery.
- Assist with staff rostering, calendar management, workshop scheduling, and oversight of day program operations.
- Monitor client progress, discharge planning, and program capacity while supporting quality improvement,



accreditation, reporting, and outcome measurement activities.
- Provide clinical and operational support for high-focus clients and assist in maintaining evidence-based and efficient service delivery practices.

Continuing Care Case Manager

- Manage a caseload of clients throughout the first 90 days of their recovery journey, providing ongoing support, motivation, and case management.
- Collaborate with multidisciplinary teams to develop, implement, and review individualised continuing care plans.
- Conduct regular client engagement, follow-up, and recovery planning sessions, ensuring clients remain connected to recommended programs and support services.
- Facilitate multidisciplinary reviews, maintain accurate clinical documentation, and ensure compliance with organisational policies and standards.
- Support clients transitioning from inpatient treatment into day programs, family programs, and other relapse prevention services.
- Foster positive therapeutic relationships that encourage participation, accountability, and sustainable recovery outcomes.

Safety, Quality & Compliance

- Actively participate in quality improvement, workplace health and safety, and risk management initiatives.
- Maintain professional standards, ethical practice, confidentiality, and compliance with accreditation requirements.
- Contribute to a culture of collaboration, continuous improvement, client-centred care, and alignment with South Pacific Private’s mission and values.
- Monitor and promote high standards of clinical safety, service quality, and professional practice across all areas of responsibility.

What We’re Looking For

Essential

- Minimum of Bachelor’s Degree in Psychology, Counselling, Psychotherapy, or a related discipline.
- Experience working in addiction, mental health, trauma-informed care, or behavioural health settings.
- Strong case management and client engagement skills, with the ability to support clients through recovery and relapse prevention programs.
- Excellent interpersonal, written,



and verbal communication skills.
- Demonstrated ability to work collaboratively within a multidisciplinary team environment.
- Strong organisational, planning, and time management skills, with the ability to manage competing priorities.
- Intermediate computer skills and experience maintaining accurate clinical documentation.
- Commitment to skilled ethics, confidentiality, and client-centred care.

Highly Regarded

- Experience coordinating programs, services, or clinical operations within a healthcare environment.
- Knowledge of 12-Step recovery programs and continuing care models.
- Experience facilitating multidisciplinary meetings and supporting treatment planning processes.
- Strong stakeholder engagement, problem-solving, and relationship management skills.
- Ability to work independently, demonstrate initiative, and maintain professional boundaries.
- Commitment to continuous improvement, quality assurance, and workplace health and safety.
