Necessity Care Pty Ltd is an established disability services provider delivering person-centered support to people with disability. As a registered NDIS provider, the company provides a range of supports designed to assist participants with their daily living needs, personal activities, community participation, development of life skills and greater independence. The company is committed to delivering responsive and individualized support that respects each participant’s dignity, choice, preferences and personal goals. Its services require ongoing engagement with participants, families, caregivers and other relevant service providers to ensure that appropriate and coordinated support is provided in accordance with each participant’s individual needs and support arrangements.

As Necessity Care Pty Ltd continues to provide disability and community support services to its participants, there is an ongoing operational requirement for suitably skilled staff who can provide consistent, person-centered assistance, facilitate community participation and support participants in achieving their individual goals. To support the effective delivery of these services and maintain an appropriate standard of participant care, Necessity Care Pty Ltd is seeking to employ a suitably qualified Full-Time Community Worker.

About the role:

The Community Worker plays an important role in delivering person-centered support to participants of Necessity Care Pty Ltd, assisting them to achieve greater independence, participate in their communities and work towards their individual goals. As a full-time position, the Community Worker will be responsible for providing practical and social support across home and community settings, assisting participants with daily living activities, community access, social participation and the development of independent living skills. The Community Worker will support participants to build confidence, strengthen their capacity to engage with community services and activities, and maintain meaningful social connections. Through consistent, respectful and individualized support,



the position will contribute to the delivery of high-quality disability and community services and to the overall well being and independence of participants supported by Necessity Care Pty Ltd.

Key Responsibilities:

Individual Support

1. Provide compassionate, person-centered support to clients in various settings, including their homes and community environments.
2. Assist clients with activities of daily living, including personal care, meal preparation and household tasks, according to their individual needs and support requirements.
3. Promote clients’ independence, dignity, choice and participation in everyday activities.

Support Planning

1. Collaborate with clients and their families to develop and implement individualized support plans that reflect each client’s goals, preferences and needs.
2. Monitor clients’ progress and regularly review support plans to ensure they remain appropriate, relevant and effective.
3. Identify changes in clients’ circumstances or support requirements and communicate these to relevant stakeholders.

Community Engagement

1. Assist clients to access community resources, services and activities that promote social inclusion, independence and personal development.
2. Encourage and support clients to participate in recreational, educational, social and vocational activities appropriate to their interests, goals and capabilities.
3. Support clients in developing confidence and skills to participate more independently within the community.

Advocacy

1. Advocate for the rights, choices and individual needs of people with disabilities and support their participation in decisions affecting their lives.
2. Provide clients and their families with appropriate information about available services, programs and community resources.
3. Support clients to communicate their preferences, concerns and support requirements to relevant service providers.

Collaboration

1. Work collaboratively with multidisciplinary teams, including healthcare professionals, therapists,



support coordinators and other service providers, to ensure coordinated and holistic support for clients.
2. Maintain effective communication with clients, families and caregivers regarding progress, emerging needs and changes in support requirements.
3. Participate in relevant meetings and case discussions as required.

Documentation and Reporting

1. Maintain accurate, confidential and up-to-date records of client interactions, services provided, incidents, observations and progress in accordance with organizational policies and procedures.
2. Prepare progress notes, reports and other documentation required by the organization, funding bodies and relevant regulatory requirements.
3. Report concerns, incidents or changes in a client’s well being or circumstances to the appropriate person in a timely manner.

Career development

1. Participate in ongoing training and professional development activities to maintain and enhance knowledge and skills relevant to disability support services.
2. Maintain awareness of current best practices, relevant legislation, policies and professional standards within the disability services sector.
3. Apply acquired knowledge and training to continuously improve the quality and safety of support provided to clients.

Candidate must have:

1. 1-2 years previous experience as a Community Worker or closely related.
2. Be able to work within a team to continue to meet the high standards expected excellent communicate.
3. Relevant qualification of industry skills or proven experience
4. Strong management and planning skills
5. Attention to detail
6. Understanding of performance marketing metrics, reporting, and optimization techniques.
7. Strong organizational skills with the ability to manage multiple priorities and deadlines.
8. Analytical mindset with attention to detail and problem-solving skills.
