Job Description
Morning, Afternoon, Weekend and Active Night Shifts Available

Are you a compassionate and experienced Disability Support Worker looking for meaningful and rewarding work?

Meraki Care Services is seeking reliable, skilled and participant-focused Disability Support Workers to join our growing team.
n
We are looking for workers who can confidently provide high-quality support while promoting each participant's independence, dignity, choice and control.
n
A variety of morning, afternoon, evening, weekend, public holiday and active night shifts may be available. Applicants with flexible availability across multiple shift types will be highly regarded.
n
About the Role
n
Depending on the participant's individual needs, your responsibilities may include:
n
n
- Supporting participants with their daily routines
n
- Providing personal care, toileting and continence assistance
n
- Assisting with meal preparation, nutrition and hydration
n
- Supporting medication administration within your training and authorisation
n
- Supporting community access, appointments and social activities
n
- Assisting with household tasks and maintaining a safe environment
n
- Providing emotional support and positive engagement
n
- Following individual support plans, health plans and risk assessments
n
- Monitoring and reporting changes in health, behaviour or wellbeing
n
- Responding appropriately to behaviours of concern
n
- Using positive behaviour support and de-escalation strategies
n
- Completing accurate progress notes, charts and incident reports
n
- Providing explicit handovers to other workers
n
- Escalating safety, health or medication concerns promptly
n
- Experience supporting participants with dementia, psychosocial disability, complex personal care, epilepsy, mobility limitations or behaviours of concern will be highly regarded.
n
- Workers rostered for active night shifts must remain fully awake, alert and available throughout the entire shift.
n

About You
n
n
- Have previous experience working as a Disability Support Worker
n
- Are compassionate, patient, reliable and respectful
n
- Can work independently and as part of a team
n
- Communicate clearly with participants, families and colleagues
n
- Can follow detailed support plans and workplace procedures
n
- Have strong observation, documentation and reporting skills
n
- Can maintain professional boundaries and participant confidentiality
n
- Are committed to participant choice, dignity and independence
n
- Can respond calmly and safely in challenging situations
n
- Are dependable and committed to their rostered shifts
n
n
Mandatory Requirements
n
n
- Certificate III in Individual Support, Disability or a related qualification
n
- Current NDIS Worker Screening Clearance
n
- Current Working with Children Check/Blue Card, where required
n
- Current First Aid and CPR certificates
n
- NDIS Worker Orientation Module certificate
n
- Reliable transport and appropriate vehicle insurance
n
n
