About the Role:

We are looking for an experienced Care Coordinator to join our Adelaide team, with strong Support at Home experience and a solid understanding of CHSP/CHIS funding and service delivery requirements.

This is a hands-on coordination role where you will support clients, coordinate services and work closely with Care Partners, Team Leaders and frontline teams to ensure services are delivered safely, efficiently and in line with funding and compliance requirements.

Key Responsibilities

- Coordinate Support at Home services and ensure clients receive timely, quality care.
- Apply your knowledge of CHSP/CHIS funding, budgets and service requirements to day-to-day coordination.
- Maintain accurate client records, care plans, service agreements and documentation.
- Coordinate client onboarding, service commencement and changes to care arrangements.
- Monitor service delivery, funding utilisation and client needs.
- Work closely with Care Partners, Team Leaders and care teams to resolve issues quickly.
- Ensure services and documentation meet compliance and quality requirements.
- Provide responsive and professional support to clients, families and key stakeholders.
- Manage competing priorities in a fast-paced aged care environment.





About You will have hands-on experience working within Support at Home or the previous CHSP environment and understand how aged care funding translates into practical service delivery.

You will also have:

- Strong knowledge of CHSP/CHIS funding and aged care service requirements.
- Experience in a Care Coordinator, Service Coordinator, Client Services or similar role.
- Strong understanding of care plans, budgets, client records and compliance.
- Excellent communication and stakeholder management skills.
- Strong attention to detail and the ability to manage competing priorities.
- Experience using aged care, client management or rostering systems.
- A genuine commitment to person-centred care.

Aged care experience, particularly Support at Home experience, is essential for this position.
