What you'll be doing
To provide safe, compassionate and high quality person centred assessment and case coordination to clients.
Ensuring care is delivered in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).
This is a temporary opportunity with possibility of ongoing employment
About You
You are a dedicated and compassionate health professional with recognised qualifications in social work, occupational therapy, physiotherapy, psychology, nursing, or a related discipline, and a strong commitment to supporting older people to maintain independence, wellbeing, and quality of life. Drawing on your experience across acute and community healthcare settings, you deliver comprehensive, person-centred assessments and develop tailored care strategies that address the complex health, functional, and social needs of older individuals and their families.
You have a strong understanding of geriatric care, chronic disease management, rehabilitation, and reablement principles, enabling you to support clients in achieving the best possible outcomes while maximising their independence and participation in daily life. Your clinical expertise includes working with people experiencing complex comorbidities, dementia, frailty, and end-of-life care needs, allowing you to confidently navigate challenging situations and provide holistic,
evidence-based support.
A skilled communicator and collaborative practitioner, you build strong professional relationships with clients, carers, general practitioners, community providers, and multidisciplinary healthcare teams. You are adept at identifying key issues, coordinating services, and facilitating effective communication to ensure seamless care delivery and positive outcomes for those you support.
Highly organised and technologically proficient, you are experienced in using a range of clinical information systems, data management platforms, and Microsoft Office applications to support assessment, care planning, documentation, and service coordination. With a current unrestricted Australian Driver Licence and a willingness to travel, you are committed to providing accessible, high-quality aged care services across a range of community and healthcare settings.
