About the role

The Clinical Liaison RN plays an important role in coordinating resident care and supporting positive clinical outcomes across the residence. A key focus of the role is coordinating GP rounds, including preparing for rounds, following up on actions and ensuring recommendations are communicated and implemented appropriately. You will also play an important role in supporting new admissions, including meeting with families, reviewing admission information and documentation, and making sure the care team is well prepared when a new resident arrives. The role also coordinates and oversees admission checklists across nursing, care, lifestyle and physiotherapy teams.

Key responsibilities

- Coordinate and prepare for regular GP rounds and follow up on outcomes and recommendations.
- Liaise with GPs, Nurse Practitioners, allied health professionals, residents and families.
- Support the admission of recent residents and help ensure a smooth transition into the residence.
- Coordinate admission checklists across the care team.
- Complete and review resident assessments and care plans as care needs change.
- Identify and respond appropriately to clinical deterioration and escalate concerns when required.
- Support safe medication management,



wound care and other appropriate clinical procedures.
- Monitor clinical documentation and ensure records are accurate, timely and complete.
- Participate in care conferences, resident meetings and clinical discussions.
- Provide guidance, support and education to other members of the care team.

About you

- Current unrestricted registration with AHPRA.
- Current Police Check.
- Current required vaccination certificates, including influenza and COVID-19.
- Appropriate Australian working rights.
- Knowledge of the Aged Care Quality Standards.
- Strong clinical assessment and care planning skills.
- Communicates clearly and professionally with residents, families and healthcare professionals.
- Can work independently while also being a strong team member.
- Organised and able to coordinate multiple clinical priorities.
- Confident identifying changes in a resident's condition and escalating concerns appropriately.

Weeroona Aged Care Residence is seeking an experienced Registered Nurse to join our multidisciplinary team alongside RNs, ENs, carers, physiotherapists, lifestyle staff, allied health professionals, GPs and Nurse Practitioners.
