About the role
Located in Kununurra, the Continuing Care Support Worker provides follow-up and coordinated support to clients after residential rehabilitation to help maintain recovery, stability and connection to appropriate services. The role works with clients to identify ongoing needs, support care plans, coordinate referrals, follow up appointments and encourage positive engagement with family, community and support networks. The position promotes culturally safe, practical and client-centred support, maintains accurate records and works closely with the Continuing Care team to ensure each client receives appropriate ongoing care.
About you
- Demonstrated relevant experience in continuing care, alcohol and other drug, community services, health, homelessness or related support work
- Sound organisational, communication and recordkeeping skills
- Ability to coordinate follow-up support, referrals, appointments and care planning in response to individual client needs
- Sound knowledge and ability to use a client management system to record accurate client information and progress notes
- Ability to work effectively as part of a team and contribute to positive, respectful and collaborative workplace relationships
- Demonstrated understanding of Aboriginal culture, community protocols and culturally secure ways of working.
- Ability to work effectively, safely and collaboratively within organisational policies, procedures, professional boundaries and confidentiality requirements
- Demonstrated ability to work respectfully with Aboriginal people, families and communities
- Current first aid certificate, current driver's licence,
Working with Children Check and recent National Police Clearance no older than six months, or ability to obtain these as required
