You will lead a multidisciplinary team to deliver holistic care, partner with residents, families and carers, and ensure care plans reflect residents' goals.This permanent full-time role calls for RN registration with AHPRA, solid leadership, and experience in care planning, with a focus on resident-centered care and seamless transitions within the home.
