West Moreton Health in Queensland is seeking a Clinical Nurse to provide advanced nursing practice within the Diabetes Service, in both outpatient and inpatient settings.
You will apply your nursing expertise to manage care plans and deliver patient-centered outcomes.You will work closely with the unit team, consumers and carers, general practitioners and partner agencies, ensuring seamless care transitions and adherence to qualified standards.
AHPRA registration is required.
