The role involves travel around Northern Sydney, conducting comprehensive aged care assessments, and determining eligibility for home support services using the Integrated Assessment Tool (IAT).
Successful candidates will have a health-related qualification, AHPRA registration, a valid Australian driver's license, and willingness to obtain a Police Check.
