About the Role Key Accountabilities include but are not limited to: Provide person-centred outreach support across the ACT community.
Build trusting relationships with participants following a suicidal crisis.
Support individuals to identify goals and access the services and supports they need.
Work collaboratively with mental health, housing, health, AOD, and community service providers.
Complete assessments, safety planning, case notes, and reporting requirements.
Deliver support using recovery-oriented, trauma-informed, and strengths-based approaches.
