Job Description
ACH Group in Mile End, South Australia, is seeking a full time Care Coordinator to support older people in the community and enhance their quality of life.

You will work closely with a multidisciplinary team to tailor Home Care Packages, perform assessments, develop and review support plans, implement care interventions, and liaise with carers, health professionals and community services to promote independence and wellbeing.
