About the role
n
The Royal Melbourne Hospital (RMH) is seeking an experienced Grade 3 Social Worker to join our highly regarded Geriatric Evaluation and Management (GEM) Program at Royal Park Campus.
n
This is an opportunity for an accomplished clinician and emerging leader to make a meaningful difference for older people, supporting them to maximise independence and achieve their goals. Working within a contemporary "home first" philosophy of care, you will provide advanced clinical social work while contributing leadership, mentoring and expertise to a multidisciplinary team.
n
You will also contribute to service development, quality improvement and evidence-based practice, helping shape the delivery of high-quality, person-centred care for older people and their families.
n

Key Accountabilities
n
n
Provide advanced psychosocial assessment, intervention and counselling for older people and their families.
n
Support patients and carers to navigate complex health, social and discharge planning needs.
n
Provide senior clinical leadership, expertise and guidance within the GEM multidisciplinary team.
n
Supervise, mentor and support junior social workers and students.
n
Contribute to service development, quality improvement and evidence-based practice.
n
Collaborate with geriatricians, nurses, allied health clinicians and care coordinators to support person-centred care.
n
Advocate for older people and their families, supporting wellbeing, independence and meaningful outcomes.

About You
n
You're an experienced Social Worker who brings strong clinical expertise, leadership and a commitment to improving outcomes for older people and their families. You'll bring:
n
n
A Social Work qualification and eligibility for membership of the Australian Association of Social Workers (AASW)
n
Significant clinical experience (minimum 7 years), ideally in sub-acute hospital or aged care settings
n
Strong psychosocial assessment, counselling and clinical reasoning skills
n
Demonstrated leadership, mentoring and supervision experience
n
Proven ability to work collaboratively within a multidisciplinary team
n
A person-centred approach and ability to build strong relationships with patients, carers and colleagues
n
Confidence navigating complex situations, influencing practice and supporting positive outcomes
n
A commitment to continuous improvement, professional development and service excellence
