HammondCare seeks a Dementia Support Coach to assist people living with dementia transitioning from hospital to home or residential care. The role offers growth, learning, and involvement in a current DC initiative from our Hervey Bay office in Queensland.
The position is a fixed-term contract until June **** with flexible hybrid work. You'll work with teams to implement care plans, coach staff, and support carers across multiple settings. A relevant qualification is desirable.
