About the Role

Join CO-ACT at Hampstead Rehabilitation Centre and help older people achieve safe, meaningful transitions from hospital to community care.

- Assess and deliver tailored physiotherapy for complex patient needs and support complex discharge planning
- Manage a varied caseload with growing clinical independence
- Partner with multidisciplinary teams to support quality patient care
- Educate colleagues and provide clinical guidance to less experienced staff
- Contribute to quality improvement, research and community care planning

About You

We're looking for a skilled physiotherapist who brings sound judgement, teamwork and a commitment to quality patient care.

- Physiotherapy degree eligible for AHPRA registration
- Experience in acute or rehabilitation settings with adults
- Sound clinical judgement managing complex caseloads independently
- Strong communication, teamwork and evidence based practice skills
- Commitment to career development, service quality and safety
