About the role
Join the Post Acute Care Service (PACS) and play a key role in supporting patients as they transition safely from hospital back to their home environment. PACS provides short-term nursing and allied health intervention for people recovering from acute illness or injury, helping them regain function, build independence and avoid unnecessary hospital readmissions.
As a Clinical Nurse, you will deliver advanced clinical care across a variety of settings including patients' homes, clinics, telehealth and telephone-based services. Working within a collaborative interprofessional team, you will provide clinical leadership, patient education and care coordination while supporting evidence-based practice and contributing to quality improvement initiatives that enhance both patient and service outcomes.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Apply advanced clinical nursing expertise to assess, plan, implement and evaluate patient-centred care for consumers transitioning from hospital to home.
- Respond to changing patient needs through advanced assessment, clinical decision-making and timely nursing interventions.
- Provide education and support to patients and their families to promote independence, self-management and positive health outcomes.
- Work collaboratively with nursing, allied health and medical teams to deliver coordinated, safe and effective care across community-based settings.
- Contribute to quality improvement, evidence-based practice, career development and service enhancement initiatives within the Post Acute Care Service.
