Smart Disability Care is seeking a Community Worker to assess client needs, develop and implement support programs, monitor progress and liaise with families and service providers. This full time role is based in NSW and requires a Diploma in Community Services or related field.

You will interview clients, plan education and support programs, coordinate referrals and maintain records with a person-centred approach to promote independence and community participation.
