Community Care Foundation is looking for an experiencedParticipant Services Facilitatorto join our Participant Services team and help lead the delivery of high-quality, person-centred supports across the Fraser Coast and Wide Bay.

This is a hands-on coordination role for someone who understands that great disability services are about more than rosters and paperwork — they are aboutpeople, relationships,qualityand outcomes.

Working closely with participants, theirstakeholdersand our Lifestyle Support Workers, you will help ensure our services are safe, compliant, well-coordinated and focused on achieving meaningful outcomes for the people we support.

The Role

As a Participant Services Facilitator, you will playan important rolein the day-to-day coordination and quality oversight ofallocatedparticipant services and SIL operations.

Your responsibilities will include:

- Coordinating the effective and efficient delivery of participant services and SIL operations
- Working collaboratively with participants and stakeholders to support NDIS goals and positive outcomes
- Building andmaintainingstrong relationships with participants, families,stakeholdersand support teams
- Communicating proactively,



professionally andin a timely mannerwith participants,stakeholdersand team members,recognising thattimelycommunication is essential to quality service delivery
- Providingday-to-day guidance, supervision and support to Lifestyle Support Workers and Senior Support Workers
- Supporting staff training, development, mandatorytrainingand competency requirements
- Working alongside Rostering tomaintainappropriate staffinglevels and continuity of support
- Proactivelymonitoringand managing overtime
- Reviewing participant documentation, support plans, health passports, riskassessmentsand allied health plans
- Conducting audits and quality checks to ensure documentation and service delivery meet required standards
- Supporting incident investigations and ensuring reports are completed within requiredtimeframes
- Participating in recruitment, onboarding and offboarding of participant services and staff
- Supporting performance and conflict management in partnership with HR and the Team Leader
- Facilitating and attending team and stakeholder meetings
- Approving staff timesheets within required payrolltimeframes
- Participating in a7-day rotating after-hours on-call roster
- Providing direct participant support whenrequired

The position descriptionidentifiesthe role as having responsibility for effective participant service and SIL operations, staff support, compliance, quality, stakeholderrelationshipsand continuous improvement.

- Takes initiative anddoesn'twaitto be asked to solve a problem

- Relevant experience in the disability, communityservicesor similar sector
- Understanding of NDIS requirements and person-centred practice
- Ability to obtain and maintainanNDIS Worker Screening Check
- Ability to obtain andmaintainaBlue Card
- CurrentQLD Driver's Licence
- Ability toparticipatein a7-day rotating after-hours on-call roster
- Relevant qualifications in Community Services, Disability, Leadership/Managementor a related field will be highly regarded

Highly Regarded

- Experience in a coordination, senior support, teamleaderor supervisory role
- Experience within SIL or other NDIS-funded services
- Experience supervising and supporting frontline staff
- Knowledge of participant documentation, audits, incidentmanagementand compliance
- Experience with rostering and workforce coordination
- Experience managing overtime and staffing requirements
- Strong understanding of Positive Behaviour Support and restrictive practices
- Experience working collaboratively with allied health professionals and other stakeholders
